El documento contiene una copia de un manuscrito y tres sobres.
La copia del manuscrito corresponde a un cuaderno en el cual el General Tomás Iriarte escribió su edad, fecha y lugar de nacimiento y también la de su esposa, sus hijos, sus nietos y sus ahijados de bautismo.
Al final del documento se encuentran tres sobres vacios, dos de ellos lacrados. Uno de los sobres se encuentra firmado por el Presidente de la República.
